Teen Sexual Health Survey – deadline December 4

WANTED:

YOUTH 15-19
WHO WANT
THEIR VOICES HEARD!

Please direct your networks, work groups, councils, and particularly YOUTH ages 15-19 to the link below.

The Colorado Organization on Adolescent Pregnancy, Parenting and Prevention is gathering youth input and feedback about topics related to teen sexual health. The purpose of the survey is to gain a better understanding of how Colorado youth view the delivery of sexual health information and what adults can do to better address this issue. The survey is targeted toward middle and high school-aged Colorado youth and should take about 20 minutes to complete. We highly encourage you to pass this survey on to young people across Colorado and encourage them to get their voices heard.

Before beginning, it’s important to know a few things:

Since this input from youth will aid in the development of a future state plan to improve teen sexual health in Colorado, it is important that youth (ages 15-19 years) from various backgrounds, cultures and experiences complete this survey. Honest and thoughtful answers are important and useful. Everything filled out in the survey will remain anonymous and confidential, and will not relate back to the individual who completed it. The data collected will be analyzed and compiled into a report that will be available online at www.healthyyouthcolorado.org.

Within the survey, there are many terms that may or may not be familiaR. We highly recommend going to www.healthyyouthcolorado.org for
survey-specific definitions that should be reviewed before beginning the
survey and can be referred back to during the survey.

After reading through the definitions, access the survey by clicking on
http://www.zoomerang.com/Survey/?p=WEB229RUV3369E.

The survey will close at 5:00 p.m. on Friday, December 4, 2009.
